We have constructed a form (please allow a few seconds for the form to load into your browser window) to encourage the reporting of any type of incident or accident whether it involves an auto, another bicycle, dogs, a road or greenway hazard. We need to know about these incidents so we can advocate for improvements and we welcome participation by the general public who are willing to improve our community.

This form can be completed using your computer then emailed to us thus saving time, paper, and postage costs. To do this, you need the latest version (8) of the free Acrobat^® reader software .
